One billion people, obese
NCD-RisC: 878 million adults lived with obesity in 2022, up from 194 million in 1990. Add children and the World Obesity Federation’s 2026 wrap is past one billion. The shot that bent the curve is still a rounding error on this line.

A billion is a market, a payer crisis, and a map
The NCD Risk Factor Collaboration’s 2024 Lancet paper is the adult count: 878 million in 2022, against 194 million in 1990. That is a 4.5-fold rise in a generation. The World Obesity Federation, reading the 2026 NCD-RisC vintage, says more than one billion people — adults and children — now live with obesity, and almost three billion with overweight or obesity. High-income prevalence is plateauing in some countries (Spain, Italy; a Nature 2026 velocity paper). The acceleration has moved to developing nations. This is no longer a ‘West’ chart.
The shot is real. It is not this line
Pair this with The shot that bent the curve. IQVIA had obesity medicines at $66 billion. Wegovy and Mounjaro changed the US tape and a dozen consumer categories. They have not yet changed a billion-person prevalence line that took thirty-two years to draw. Manufacturing, cold chain, payer rules and a still-large list of people who will not, or cannot, inject, are why a drug class can be the most important consumer story of the 2020s and a rounding error on a UN-scale body-mass chart. The 2026 Nature paper’s other sentence is the one investors skip: in much of the rich world the rise has already slowed. The next hundred million are not in Boston.
The long view is metabolic disease as a 2050s fiscal line
Obesity is diabetes, joints, hearts, cancers, and a labour-force that tires. It is also a manufacturing and a generics story once the patents roll. Own incretin manufacturing and the second-order consumer losers (see the shot), later-life care, and a scepticism toward any 2030 ‘obesity solved’ slide. Do not own a single-country GLP-1 royalty as if it were the 878 million.

Two billion people will be over sixty
There were a billion people aged 60+ in 2020. WHO sees 1.4 billion in 2030 and 2.1 billion in 2050. By the late 2070s the 65s outnumber the children. This is the quietest force in the issue, and the one that does not mean-revert.

UN: more than half of humanity lives in urban areas. In 1950 it was about a third. By 2050 it will be about two-thirds. The century’s demand is a skyline: housing, wires, water, heat, and a bus.
